
At your first meeting a service coordinator will explain the EI program and help plan evaluations and assessments. This is called an intake meeting because you will be asked to share information about your child and family.
- The service coordinator will ask you questions about your child’s growth and development, medical history, and your daily activities (play time, meals, sleep schedules, child care).
- You also may be given a checklist to identify your child’s skills and strengths.
- You will be asked to sign consent forms agreeing that the service coordinator may share information about your child with the team that will evaluate and assess your child and may help you plan for EI services. If you give consent, then your child can be referred for evaluations and assessments.
- This process is free for all families.
